Former House Speaker Nancy Pelosi has ignited fresh political fire after declaring she’s fighting to legalize sex-change surgeries for minors nationwide—despite President Trump’s sweeping executive order banning such procedures.
Speaking outside the San Francisco VA Medical Center during a Thursday press conference, Pelosi didn’t hold back.
“That is something I’m working for at the national level,” she said when asked about California’s recent pause on transgender treatments for youth. “We are hoping we can have gender-affirming care for our trans kids. It’s a sad thing for us.”
Pelosi added that the transgender pride flag hangs outside her office and those of several other Democrats in solidarity with trans youth. “It’s about visibility and rights,” she said.
But critics see it differently—especially inside the Trump administration.
Pelosi’s remarks came just months after President Trump signed Executive Order 14095 on January 28, banning all federal funding and support for sex-change procedures and cross-sex hormone treatments for individuals under 19.
The order directed the Department of Justice to investigate doctors and clinics that offer such services to minors. Attorney General Pam Bondi followed up with a forceful memo in May urging federal prosecutors to “vigorously pursue any violations of the order.”
Assistant Attorney General Brett Shumate echoed the stance, calling the treatments “experimental, irreversible, and medically unnecessary.”
In response, a coalition of 17 Democrat-led states filed a federal lawsuit last week, accusing the Trump administration of “medical censorship” and “threatening doctors with baseless criminal charges.” The case, filed in Massachusetts federal court, targets Trump, Bondi, and the DOJ directly.
California, New York, Illinois, and Michigan are among the plaintiffs. All currently allow some form of medical transition for minors.
“The government should not be in the business of controlling how doctors treat their patients,” said Massachusetts Attorney General Andrea Campbell, who is leading the legal charge.
Pelosi confirmed that her national effort to support sex changes for minors is tied to this multi-state lawsuit.
Meanwhile, fallout from the Trump administration’s crackdown is already being felt on the ground.
- Kaiser Permanente announced it will pause all gender-transition surgeries for minors beginning Aug. 29.
- Children’s Hospital Los Angeles, once a leader in trans youth healthcare, shut down its Center for Trans Youth Health and Development in July.
- Children’s National Hospital in Washington, D.C. said it will no longer offer gender-related medical interventions to patients under 19.
These announcements came shortly after DOJ letters were sent to several institutions warning of criminal liability for violating the executive order.
Supporters of the president praised the order as a necessary step to “protect children from irreversible harm.”
“The idea that a child should be able to make life-altering medical decisions before they’re even old enough to vote or join the military is insane,” said Rep. Marjorie Taylor Greene (R-GA), a vocal advocate of the administration’s stance.
But Pelosi and other Democrats argue the issue is one of civil rights.
“This is about human dignity,” Pelosi insisted. “We’re not giving up.”
The court battle over transgender medical care for minors is expected to reach the Supreme Court, with both sides preparing for what could be a defining decision on the limits of federal power over medicine—and the rights of transgender youth.
For now, the transgender flag still flies outside Pelosi’s office on Capitol Hill. But with Trump in the White House and his DOJ aggressively enforcing the new order, the future of trans healthcare in America remains more uncertain than ever.
